About AnestSim
AnestSim is a real-time educational anesthesia simulator, available globally. It was born inside an anesthesiology residency and it grows with it.
The author
AnestSim is written by a second-year anesthesiology resident (R2) at UNIFESP/EPM. The project started as an HTML file to study physiology and pharmacology outside the operating room. Each physiological and pharmacological parameter has a reference: Stoelting, Miller, Barash, or the original paper. When a value is a teaching calibration with no numeric source, the code says so.
The project
The simulator models a whole patient: heart, lungs, gas exchange, baroreflex, blood volume, neuromuscular blockade, and anesthetic depth, calculated every 0.1 second. The drugs follow compartmental pharmacokinetic models, with effect-site concentration, TCI, and interactions between hypnotics and opioids. Regional anesthesia includes spinal anesthesia, epidural anesthesia, and peripheral nerve blocks, with a map of dermatomes, nerves, muscles, and bones. At the end of each case, a technique-specific report shows what went well, what could be better, and the debriefing questions.
Three product lines use the same clinical engine:
- AnestSim Play, a game with missions, levels, scores, and leaderboards, for PC, consoles, and phones.
- AnestSim Training, the simulator for residents and anesthesiologists, with the real cases, the report, and the history.
- AnestSim Teach, the educator dashboard, with classrooms, assignments, a report for each student, and the live console.
The rewrite
The first version of AnestSim lived in a single HTML file, with 67 published versions. That version is now the oracle: the new architecture reproduces its signals second by second, in parity tests. The rewrite separates the physiology engine, the anesthesia plugin, the case content, the interface, and this portal. The engine runs on the server and the interface only draws. The clinical content lives in schema-validated data files. This separation makes the game, the educator dashboard, and the automated tests possible.
Educational notice
Educational tool, not validated for clinical use. AnestSim helps you learn and teach. It does not replace clinical judgment, the institution's protocols, the literature, or the supervision of a preceptor. No course of action from the simulator may be applied to a real patient without the assessment of a licensed physician.
