Inside the engine
How it works
AnestSim simulates a whole patient, not a list of questions.
- Physiology engine
- Heart, lungs, gas exchange, baroreflex and blood volume, calculated every 0.1 seconds.
- Real drugs with PK/PD
- Propofol, opioids, neuromuscular blockers, volatile anesthetics and vasoactive drugs, with model-based pharmacokinetics and pharmacodynamics.
- Schnider, Marsh and Eleveld models for propofol. Minto for remifentanil.
- Regional anesthesia
- Spinal, epidural and peripheral nerve blocks, with a map of dermatomes, nerves and surgical coverage.
- Report and debriefing
- At the end of the case, a report tells you what went well and what could be better, and asks the debriefing questions.

Missions, levels, scores and leaderboards on top of the same clinical engine. Nothing is scripted. Everything is physiology.

The monitor tells the story
ECG, capnography, blood pressure and saturation change with what you do. One bolus too many shows up in the blood pressure. One breath too few shows up in the CO₂.
- One bolus too many. The pressure drops over the next beats.
- One breath too few. The capnogram goes flat for one cycle.

Frequently asked questions
Scientific foundation
Every physiological and pharmacological parameter has a reference: Stoelting, Miller, Barash or the original paper. When a value is a teaching calibration with no numeric source, the code says so.
Made by someone in the operating room
AnestSim is written by a second-year anesthesiology resident (R2) at UNIFESP/EPM, from the real questions of residency. Every physiological and pharmacological parameter comes with the reference behind it.
